Event Description

A FREE educational event for youth ages 14-21 and their parents/caregivers
For youth (and their parents) who have a chronic health condition, disability or special need, and is thinking about or has already started the transition into adult health care.

Youth: Come talk with other youth and learn how to become more independent with your health care. You can choose to learn about basics like how to make a doctor's appointment, ordering your own prescriptions and how to prepare and participate in clinic visits. You could also explore more advanced topics like what is expected legally after you turn 18, how to manage medical needs at college and how to manage your health while working toward your life goals.

Parents: Come talk with other parents and learn about the healthcare transition process. You will learn ways to support your child and explore issues such as health insurance, social security, guardianship and healthcare power of attorney
